The inspector’s account

On 04/17/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Administrator Paul Michaels who was informed of the purpose of the visit. Licensee Sameh Michael was also present in the facility during the annual inspection.

The facility has a fire clearance to serve four (4) ambulatory adults. During today's visit, LPA did not observe any clients present and was informed all four (4) clients were out in the community attending day program. LPA toured the facility with Administrator Michaels and observed the facility is made up of a one-story home with four (4) client bedrooms, two (2) bathrooms, a kitchen, dining area, living room, laundry room, and attached garage. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. Client bedrooms had the required bedding, furniture, and lighting. Bathrooms had nonskid mats in the showers. No bodies of water were observed on the premises. The facility had a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications are secured in a locked kitchen closet. Administrator tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. LPA also observed a charged fire extinguisher mounted near the kitchen. Client files reviewed had all Departmental required records. LPA reviewed the facility's Fire Drill log noting their last emergency drill was conducted on 4/1/2026. LPA also reviewed the Record Of Client's/Resident's Safeguarded Cash Resources (LIC 405 ) while administrator counted their physical monies and no discrepancies were discovered. Licensee reported there have been no changes to the licensee entity since becoming licensed. Clients' personal rights, complaint procedures and emergency disaster plan are visibly posted on the hallway wall near the kitchen. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Administrator Michaels.
