The inspector’s account

Licensing Program Analysts (LPAs) Janira Arreola and Janette Romero conducted an unannounced annual required visit. LPA was granted entry and met with Staff, Genelle Crouch, who was informed of the purpose of the visit. At the time of the visit there were (2) staff and (3) clients present.

The facility is a one story home with (5) bedrooms and (3) bathrooms. No pools or firearms are housed at the facility. The facility is approved for (5) ambulatory clients. (1) client was identified as non ambulatory, however their Individualized Program Plan (IPP) and prior LIC602 Physician's Report indicated the client in ambulatory. LPA observed the client is ambulatory during the time of the visit, there a technical note was issued to seek clarification on the documented ambulatory status of the client.

Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair were present. The outdoor area had seating and shade for client, and was free of hazards. LPA observed hygiene supplies for residents, PPE, clean linens. There are designated locked areas for sharps, dangerous objects, cleaning supplies, and medications. The smoke detector and carbon monoxide are operational. The hot water temperature which read 105F. There are emergency supplies such as emergency lighting, complete first aid kit and emergency food and water. The emergency exits paths are clear and free of obstructions. LPA observed facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods.

LPA reviewed the facility's emergency and disaster plan and infectious control plan. LPA reviewed documentation showing the facility's last fire drill 03/03/2026, which met the department requirements. Required postings were found in the facility. The listed administrator has a current administrator's certificate. The facility has a staff schedule with fingerprint cleared staff. LPA reviewed (3) staff files and training. All staff have criminal clearance and updated training along with CPR/First Aid Certification.
