Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola, conducted an unannounced visit to the facility in order to investigate the above allegations. LPA met with Staff, Nosiphesihle Mdzinso, who was informed of the purpose of the visit. During the visit, LPA conducted interviews, conducted a walk through, and conducted records review.

LPA conducted a records review for (2) clients who did not have a medical assessment done prior to placement by a medical professional. This deficiencies was cited and a plan of correction was created. An exit interview was conducted where this report, LIC809 D page and appeal rights were reviewed and provided.
