Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced Case Management Visit. LPA met with Administrator/ Licensee Taye Dodo- Williams identified herself and informed on the purpose of the visit. During the time of the visit there were (2) staff and (1) clients present.

The Case Management Visit in regards to Substantiated complaint Reference Number #56-AS-20260309112858. LPA discussed licensing topics contained in 80001 Definitions and advised the Licensee that community care facility licenses are non-transferable. LPA also discussed licensing topics in 80064(a)(3) Administrator - Qualifications and Duties were also discussed, Administrator shall have knowledge and comply with applicable laws and regulations.

Technical Support Services (TSP) were offered to the licensee who expressed interest in the program. A referral for services will be completed. LPA also provided website information on where the Licensee can access TSP guides. A copy of the LIC311 was provided during the time of the visit.

An exit interview was conducted where this report was reviewed and provided.
