Facility Evaluation Report
Licensing Program Analyst (LPA), Janira Arreola, conducted an unannounced annual required inspection and met with Administrator, Ngozi Atufunwa. There was (1) staff and (0) clients at the time of the visit.
The facility is an Adult Residential Facility (ARF) licensed for four (4) ambulatory clients. No pools or fire arms are currently being kept at the facility. There are currently no clients in care. The home is composed of four (4) client bedrooms and three (3) bathrooms, with attached garage. The interior and exterior walkways of the home were observed free of obstructions. Smoke and Carbon Monoxide detectors are in working condition. The physical plant is clean and in good repair. Fixtures and furniture were present for future clients including hygiene items, extra linens and towels. Outdoor areas had sufficient room for activities and leisure. A washing machine and dryer were available and in working order. Medications, house hold cleaners and kitchen knifes will be stored in a locked areas inaccessible to clients. A complete first aid kit was observed to be available.
No immediate health or safety issues were observed. No deficiencies were cited at the time of the visit. An exit interview was conducted with the administrator where this report was reviewed and provided.
