Facility Evaluation Report

On 07/21/2026 Licensing Program Analyst (LPA) Aziz Faizi made an unannounced visit to the facility to conduct a follow-up case management incident visit. LPA met with administrator Hannah Jite and caregiver Solomon Blay and were informed of the purpose of the visit.

During the visit, LPA toured the facility, conducted staff and client interviews, and requested staff and residents' documents from Hannah Jite.

LPA observed residents Kendyjon Holes and John Brags being in their rooms, either playing games or sleeping. Resident Benjamin Calixto was noted to have left the facility for a dentist appointment with caregiver Daniel Musa.

LPA conducted a health, safety and welfare check for residents in care and did not observe any health or safety concerns at the time of the visit.

Caregiver Solomon Blay and administrator Hannah Jite were the only staff members present in the facility at the time of the visit.

An exit interview was conducted, a copy of this report was provided to the Administrator Hannah Jite.
