Facility Evaluation Report
On 06/15/2026 at 9:30 AM Licensing Program Analyst (LPA) Aziz Faizi made an unannounced visit to the facility and met with Licensee Chyke Ehiemere to conduct a case management incident visit. LPA informed Licensee Chyke Ehiemere of the purpose of the visit.
During the visit, LPA toured the facility and made observations pertaining to the incident. LPA conducted staff interviews, requested and received pertinent documents and conducted record reviews.
LPA observed residents Benjamin Calixto and John Brags seated in the living room of the facility. LPA conducted a health, safety and welfare check for ttwo (2) residents in care. LPA did not observe any health and safety concerns at the time of the visit.
An exit interview was conducted, a copy of this report was provided to the Licensee Chyke Ehiemere
