Facility Evaluation Report

Licensing Program Analyst (LPA) Yolanda Delgado arrived unannounced to complete the annual inspection that was started May 7, 2024. LPA was greeted by facility staff, Mearlene Martin. There are three (3) clients reside at this facility and are present and there are currently one (1) caregiver present. Some concerns were addressed and corrected prior to LPA's visit today.

Based on the information received during this visit today in the areas reviewed, seven (7) deficiencies will be issued per Title 22, Division 6 of the California Code of Regulations.

This LIC 809, 809D and Appeal Rights report was reviewed with the facility representative and a copy was provided.
