Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola, conducted an unannounced visit to the facility in order to conduct a case management. LPA met with staff Marlene Martin who was informed on the purpose of the visit. LPA conducted a tour of the facility, conducted a records review, and interviews.

LPA observed at the time of the visit, Person #1 in the home with no cleared staff present with (1) client at the time of the visit. Cleared staff arrived (5) minutes late to the home. When cleared staff arrived the uncleared staff was escorted off the premises. While LPA waited for cleared staff to arrive, LPA observed unlocked medications on the kitchen counter. Interview with staff revealed the medication was AM medication that was refused and was not locked up.

The deficiencies and civil penalty assessment was issued at the time of the visit. An exit interview was conducted with staff who was informed of the report and provided a copy.
