Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced case management visit. LPA met with Staff, Marlene Martin, who was informed of the purpose of the visit. There were (1) staff and (3) residents present during the time of the visit. LPA conducted interviews and a walk through. No immediate health of safety concerns were observed.

LPA conducted the case management visit in order to deliver an Engagement Letter, LIC401, LIC401A, LIC403, and LIC403A from the Department’s auditing team. The original engagement letter was sent on June 11 TH 2025 however, despite efforts to deliver the letter via mail and email, the licensee expressed not receiving the documents. On today’s date the licensee was contacted via phone, and a physical copy of the documents were provided to facility staff member, Marlene.

An exit interview was conducted with the licensee Mary Martin over the phone, and with staff Marlene in person where this report and the attached documents listed above were reviewed and provided.
