Facility Evaluation Report

On 4/9/2024, Community Care Licensing (CCL) held an office meeting conducted in the Riverside Adult and Senior Care Regional Office. In attendance were Regional Manager (RM) Reyna Lacey, Licensing Program Manager (LPM) Rikesha Stamps, Licensing Program Manager (LPM) Anthony Perez, Licensing Program Analyst (LPA) Janira Arreola, Licensing Program Analyst (LPA) Debbie Palacios, Department of Labor (DOL) Daniel Pasquil District Director , Licensee Mary Martin, and the Licensee's Attorney Steve Madoni.

The purpose of the meeting was to ensure the facility is in compliance with applicable statutes and regulations. During today’s meeting it was discussed how the licensee plans to comply with DOL regarding a Settlement Agreement dated 12/23/2024. DOL representative agreed to give the licensee additional time and the licensee agreed to contact DOL within a week. CCL will follow up with DOL to determine compliance.

An exit interview was conducted with Licensee Mary Martin who was provided a copy of this report.
