Facility Evaluation Report
On 05/29/2025, Licensing Program Analysts (LPAs) Debbie Palacios and Seo Jeon made an unannounced visit to the facility to conduct a required annual inspection. LPA's arrived at 1:42 pm and no staff or clients were present at the facility. At 2:15 pm, caregiver Mearlene Martin arrived at the facility with two (2) clients. LPAs met with Caregiver Mearlene Martin who was informed of the purpose of the visit.
LPAs toured the facility and reviewed records. During the tour, LPAs observed the facility is made up of a one-story home with five (5) client bedrooms and two (2) bathrooms, a living room, dining room, kitchen, and attached garage. All client bedrooms had the required furniture, lighting, and closet storage. LPAs toured the facility's exterior and observed outdoor pathways were free of obstructions. LPAs toured the kitchen and observed the facility has a two-day supply and a seven-day supply of non-perishable foods that were observed to be properly stored and readily available. Knives and sharp instruments were secured in a locked kitchen cabinet. LPAs reviewed random client and staff records. Client files reviewed had updated Individual Program Plans and signed admission agreement. Staff files reviewed had the Department's required training records and valid first aid/CPR certification. LPAs reviewed the facility's Fire and Earthquake Drill logs dated 05/06/2025. LPA also observed detergents and cleaning supplies were stored in a kitchen locked cabinet. The facility has no record of the Infection Control Plan. Citation was issued .
Based on today's visit citations were issued on the attached 809D, in accordance with the California Code of Regulations (Title 22, Division 6, Chapter 8).
An exit interview was conducted and a copy of this report, appeal rights, LIC9098-Proof of Corrections form, were provided to Mearlene Martin.
