Facility Evaluation Report
Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced visit to the facility in order to conducted a case management on a self-reported incident. LPA met with House Manager, Maria Medina who was informed of the purpose of the visit.
An incident report for 10/26/2025 was received by the Department for an incident occurring to a client while out in the community. Client interview, staff interviews, and records review of client Individualized Program Plans (IPP)'s, Behavior Assessment, LIC602 Physician's Reports, and client incident reports were conducted. Additional review is needed of incident, and additional phone calls and or visits may be needed.
During the time of the visit, based on record review and client and staff interviews it was revealed that an incident occurring in September of 2025 involving Client #1 (C1) was not reported to the licensing agency where C1 was involved with police. Therefore, a deficiency was cited for reporting requirements as the reporting time frame of (7) days has lapsed.
An exit interview was conducted and a copy of this report was reviewed and provided.
