Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced visit to the facility for a case management visit. LPA met initally met with staff, Gene Bonifacio. LPA later met with Licensee, Mary Martin during the visit. During the visit there were (2) clients and (1) staff present.

LPA conducted a walk through and observed clients in care. LPA observed clients asleep and eating breakfast at the time of the visit. LPA checked locked items such as sharp and dangerous objects, medications, and cleaning supplies. LPA check medications for the month of October and observed they were filled and given to the clients.

No immediate health or safety issues were observed during the visit. An exit interview was conducted where this report was reviewed and provided.
