Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced visit to the facility in order to conduct a case management visit. LPA met with Staff, Gene Bonifacio who was informed on the purpose of the visit. LPA observed (1) staff and (2) clients during the visit.

LPA conducted a walk through and observed clients in care. LPA observed locked medications, food supply and working utilities in the home. LPA observed staff present is fingerprint cleared, and resident rooms are in good repair. LPA observed no immediate health or safety issues at the time of the visit. An exit interview was conducted where this report was reviewed and provided.
