Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced annual required visit. LPA was granted entry and met with Administrator, Linda Thomas and Erik Thomas who were informed of the purpose of the visit. At the time of the visit there was (3) staff and (8) residents present.

The facility is a one story home with (6) bedrooms and (5) bathrooms. No pools or firearms are housed at the facility. The facility is approved for (8) non-ambulatory residents. Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair and were present. The outdoor area had a shaded area and seating for client, and was free of hazards. LPA observed hygiene supplies for residents, PPE, clean linens. There are designated locked areas for sharps, dangerous objects, cleaning supplies, and medications. The carbon monoxide detectors and smoke detectors were operational, and the hot water temperature 109.4F. There are emergency supplies such as emergency lighting, complete first aid kit and emergency food and water. The emergency exits paths are clear and free of obstructions. LPA observed facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods.

LPA reviewed the facility's emergency and disaster plan and infectious control plan. LPA reviewed documentation showing the facility's last fire drill 02/13/2026, which met the department requirements. Required postings were found in the facility. The listed administrator has a current administrator's certificate. The facility has a staff schedule with fingerprint cleared staff. LPA reviewed (4) staff files and training. All staff have criminal clearance and updated training along with CPR/First Aid Certification.
