The inspector’s account

On July 31,2026, Licensing Program Analyst (LPA), Venus Mixson at the facility without prior notice and met with the Administrator, Lourdes Abella. LPA introduced herself and stated the purpose of visit. The facility is approved to care for six ambulatory Adults and serves adults ages 18-59, the Licensee is operating within the scope of the license at this time. LPA toured the facility along with the administrator. LPA observed kitchen to be clean. Food is stored in a safe and healthful manner. Utensils and dishware are sufficient for the approved capacity. LPA observed food supply met the requirement for a two-day supply of perishable food and seven-day supply of non-perishable food. Knives/sharps are secured in a locked kitchen closet. LPA toured the dining and living/family room area.

LPA observed area to be clean and furniture in good repair. Medications were locked and in their original containers currently. Activities and board games are available for clients in care. LPA observed hallway to be clean with no pathway obstructions. Carbon monoxide & smoke detectors were tested and functioning properly. Fire extinguisher is charged and mounted near entrance hallway. The facility phone number is (951) 485-4230 and it is operable. LPA Mixson observed the residents’ bedrooms, and each was equipped with required furniture as per Title 22. Administrator certificate is current. LPA Mixson inspected facility bathrooms, and the hot water temperature tested within regulations, and was logged. The bathrooms were clean, and appliances were operating appropriately at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. LPA Mixson observed required postings such as "If you See Something, Say Something,” and the "Personal Rights." The cleaning supplies and sharp items were locked and inaccessible to the residents in care at the time of this visit. There was a designated storage space for the residents and staff files, and it was locked at the time of this visit. There were no TA deficiencies observed or cited per Title 22, Division 6 of the California Code of Regulations at this time. An exit interview was conducted where a copy of this report was discussed and given to Lourdes Abella, Administrator.
