The inspector’s account

On 02/27/2026, Licensing Program Analysts (LPAs) Janette Romero and Kyle Wellington made an unannounced visit to the facility to conduct a required annual inspection. LPAs were greeted and granted entry by Licensee Rossano Zafra who was informed of the purpose of the visit. The facility has a fire clearance to serve four (4) ambulatory adults ages 18 through 59. Licensee reported the facility is currently unoccupied pending Inland Regional Center (IRC) vendorization.

LPAs toured the facility with licensee and observed the facility is made up of a one (1) story home with four (4) client bedrooms, two (2) bathrooms, a kitchen, laundry room, dining area, and garage. Indoor and outdoor passageways were free of obstruction. No bodies of water were observed on the premises. Licensee tested the smoke alarm/carbon monoxide detector and LPAs heard them to be operational. LPAs also observed a charged fire extinguisher mounted in the kitchen. The facility has operating utilities including gas, water, and electricity. LPAs observed a working refrigerator and storage space for a two-day supply of perishable foods and seven-day supply of nonperishable food items. Client bedrooms had the required bedding, furniture, and lighting. Bathrooms are equipped with grab bars and non-skid mats in the showers. Medications will be centrally stored in a locked kitchen closet. The facility also has a confidential storage area to secure staff and client files. Complaint procedures, facility sketch, and emergency disaster plan are posted near front entrance. During today's visit, LPAs did not observe any issues or concerns. Licensee was advised to notify Community Care Licensing when the facility becomes an IRC vendor and accepts client placement. An exit interview was conducted, and a copy of this report was reviewed and provided to Licensee Zafra.
