On 01/22/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management incident visit. LPA met with Administrator Florence Bazile, where LPA explained the purpose of the visit. On 01/16/25 the department received a Special Injury report, reported an incident which required follow up.
During today's visit LPA gathered pertinent documentation, and conducted interviews. There were no health and safety concerns observed during today's visit.
An exit interview was conducted and a copy of this report was provided to Florence Bazile, Administrator.
