Facility Evaluation Report
On 09/22/25 Licensing Program Analyst (LPA) Javina George conducted a 1 year required visit. LPA was greeted and granted entry by Jiesse Arrellano, Direct Support Professional II, where LPA explained the purpose of the visit. The facility does not have any clients in care at this time. The Administrator was unable to come to the facility and sent a staff over from the Sister facility. The facility is licensed to serve four (4) ambulatory clients ages 18 to 59. Below are the observations made during today's visit:
The facility is a single story structure consisting of four (4) bedroom, two (2) bathrooms home with a living room, dining room, kitchen, laundry room, garage and backyard. The bedrooms were observed to have the required furniture such as a bed, night stand, dresser and chair. There are no pools or bodies of water on the premises, or known guns and ammunition.
The water temperature was tested and found to be within regulatory limits measuring at 112.3 degrees Fahrenheit. The fire extinguishers were observed to be fully charged and were last serviced on 09/02/25. The smoke and carbon monoxide alarms were observed to be operable. The administrator Maria Talla was observed to have a valid administrator certification, which expires on 06/23/26. The facility's annual fees were observed to be paid, and for the governing body to be in good standing. The facility does not have a land line, nor a 2 day supply of perishable food items. Staff was reminded to purchase the minimum as soon as they are aware of any potential placement's. A reminder was given to check the nonperishable food for any expired items.
Based on today's inspection no citations were issued. An exit interview was conducted and a copy of this report was reviewed and provided to Jiesse Arrellano, Direct Support Professional II.
