Facility Evaluation Report
On 1/14/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit. LPA Flores was greeted by Administrator Arleen Sengamphan and was granted entry into the facility. LPA conducted a tour of the facility alongside Arleen and observed the following:
The facility is a two-story structure which consist of (3) three client bedrooms, (1) one client bathroom, and the second level is designated for staff only. Laundry equipment was in good working condition. Bedrooms maintained the required bedding, furniture, seating, and functional lighting. Indoor and outdoor passageways were free from obstruction. Facility maintained a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable food. Knives and cleaning supplying are kept in a locked cabinet inaccessible to clients. LPA observed emergency food and water located in the garage. Centrally stored medication was located in a locked closet. No pools or other bodies of water was observed on the property. Smoke and carbon monoxide detectors were observed to be operational. LPA toured hall bathroom and observed bathroom to be clean and equipped with grab bar and non-slip mat. There is also a good number of personal toiletries available for the residents in care. Night lights were maintained throughout the facility. LPA toured the backyard and observed backyard to be clean and furniture's in good condition. Administrator no firearms or dangerous weapons are stored at the facility.
LPA reviewed (3) three out of (3) three client records. Records review included but not limited to admission agreement, preplacement appraisal, a current IPP, personal rights, . LPA reviewed (2) two out of (3) three clients Personal and Incident (P&I) Funds and did not observe any discrepancies. Per Administrator, (1) one out of (3) three clients manage their own funds. No deficiencies noted at the time of visit.
An exit interview was conducted, and a copy of this report was reviewed and provided to Administrator Arleen Sengamphan.
