Facility Evaluation Report
On 03/11/25 Licensing Program Analyst (LPA) Javina George made an unannounced 1 year required visit. LPA was granted entry and met with Administrator Lidia Pina, who was informed of the purpose of the visit. At the time of the visit there was (1) staff and (0) clients present. The facility is a single story structure with (5) bedrooms and (6) bathrooms. There is also a trailer inside the backyard, that will not be utilized by clients. The facility is licensed to serve clients between the ages of 18-59, that are both ambulatory. The facility is currently going through the process to be vendorized with Inland Regional Center, and is awaiting clients to be placed.
The facility was observed to be clean and clutter free. The sharps and hazardous items are stored in a locked cabinet inside the pantry. The hot water was tested and measured to be at 105 degrees Fahrenheit. The facility was observed to have a fully charged fire extinguishers. The chemicals were observed to be inside a box inside the garage.
The facility was observed to have a LPA observed the facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods.
There are no pools or bodies of water, or guns and ammunition on the premises. The facility was observed to have emergency and disaster plan and emergency disaster supplies, in addition to infection control kit with supplies such as gowns, gloves, and masks.
Due to no clients in care there were no medications, Personal and Incidental (P&I) Funds, or client files reviewed. LPA reviewed staff files which were observed to have valid CPR/First Aid, as well as an active administrator certificate which expires on 09/26/26. All staff were observed to have criminal record clearance and to be associated to the facility.
There were no deficiencies cited during today's visit. An exit interview was conducted where a copy of this report was reviewed and provided to, Licensee Lidia Pina.
