The inspector’s account

On 11/25/2025, Licensing Program Analyst (LPA) Valerie Flores made an announced visit to the facility for the purpose of conducting the 1-year required annual inspection. LPA was greeted by Licensee Tasha Hawkins and was granted entry. Information received from Tasha reported that the facility has not retained any clients since being licensed. Tasha is in the process of being vendorized by Inland Regional Center and is awaiting client placement. LPA Flores observed the following during today's visit:

The facility is a single-story structure which consist of (4) four client bedrooms, (2.5) two and a half bathrooms, laundry room, kitchen, living room, dining room, garage, and backyard. The kitchen was clean and free of liter. The facility maintained the required (2) two day supply of perishable foods and (7) seven day supply of non-perishable foods. LPA observed a locked file cabinet located in the living room which housed knives and other sharp objects. LPA observed an additional locked file cabinet which is designated for centrally stored medication. The facility maintained extra linen and towels for future client use. LPA observed books and other activities in the living room intended to promote client socialization. Client bedrooms were equipped with the required bedding, furniture, seating, and functional lighting. Indoor/Outdoor passageways were free from obstruction. LPA observed a fully charge fire extinguisher mounted on the wall in the hallway. Outdoor area was furnished and Licensee provided shading to encourage outdoor socialization for future clients. The dual smoke and carbon monoxide detectors was tested and was observed to be fully operable. Per Licensee, there are no firearms and/or ammunition on the premises. The facility was in good repair and is prepared to accept client.

LPA observed the following required postings during the visit: See Something Say Something, facility license, Administrator Certification, facility sketch, client personal rights, employee personal rights, and LIC500. LPA did not observe any health and safety concerns.

An exit interview was conducted and a copy of this report was provided to the Licensee, Tasha Hawkins.
