Facility Evaluation Report
On 04/17/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Caregiver Omorodion Owens who was informed of the purpose of the visit. Licensee Vivien Owens arrived to the facility for the annual inspection. The facility has a fire clearance to serve four (4) ambulatory adults. During today's visit, LPA observed two (2) clients and one (1) staff present.
LPA toured the facility with Caregiver Owens and observed the facility is made up of a one-story home with four (4) client bedrooms, two (2) bathrooms, a kitchen, dining area, living room, laundry room, staff office, and attached garage. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. Client bedrooms had the required bedding, furniture, and lighting. Bathrooms had nonskid mats in the showers. The facility had a two-day supply of perishable foods and seven-day supply of non-perishable food items. Knives are secured in a locked box stored in the kitchen cabinet under the sink. Medications are secured in a locked filing cabinet near the staff office. LPA observed an additional supply of hygiene items available for the clients in care.Caregiver tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. LPA also observed a charged fire extinguisher mounted near the kitchen that was last serviced on 06/06/2025. Licensee reported there have been no changes to the licensee entity since becoming licensed. Clients' personal rights, complaint procedures, emergency disaster plan and facility sketch are visibly posted near the entrance. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Caregiver Owens.
