Facility Evaluation Report
On November 7, 2025, Licensing Program Analyst (LPA), Yolanda Delgado made an unannounced visit for the purpose of the facility's closure and rang the video door bell with no answer and was unable to make contact with someone at the facility, vacant.
The Licensee initiated this closure on 03/08/2025 with effective date of closure 04/21/2025. LPA Banrasavong made a visit on 03/13/2025 and was able to meet with Licensee and at the time there was still one resident in care. Licensee stated the reason for closure was due to personal reasons.
LPA made a visual observation of the exterior of the property and observed the home to be vacant by the view in the areas of the dining, kitchen and living area, no furniture observed in the areas viewed, a lockbox on the front door, a for sale sign posted in the yard. LPA conducted an address search on Zillow and home is pending sale. LPA went to address at 31257 Casera Court knocked, and observed to be vacant with no furniture through the windows and for sale sign posted in the yard by the same Real Estate Agency, LPA knocked on the door at 31225 Casera Court and there was no answer.
An exit interview was not conducted as the facility has been closed since 07/12/2025 when property was listed for sale. A copy of this report will be sent to licensee’s last known address via USPS certified mail and/or licensee’s business email on record, due to the facility closure.
