The inspector’s account

On 04/16/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Caregiver Eloisa Santiago who was informed of the purpose of the visit. Administrator Sameh Awad was notified of LPA's visit and was available via phone. The facility has a fire clearance for six (6) non-ambulatory elderly residents, of which one (1) may be bedridden. The facility also has a hospice waiver for three (3) residents and LPA was informed none of the current residents are receiving hospice services. During today's visit, there was six (6) residents and two (2) staff present. Staff present have a criminal record clearance, valid CPR/first aid certification, and are associated with the facility. Administrator reported there have been no changes in the licensee entity since becoming licensed.

LPA toured the facility with Caregiver Santiago and observed the facility is made up of a one-story home with five (5) resident bedrooms, three (3) full bathrooms, a kitchen, dining room, living room, laundry room, staff office and attached garage. Indoor and outdoor passageways are free of obstruction. No bodies of water were observed on the premises. Resident bedrooms had the required bedding, furniture, and lighting. Bathrooms had grab bars and non-skid mats in the showers. The facility had a two-day supply of perishable foods and seven-day supply of non-perishable food items. Medications are secured in a locked kitchen cabinet. Caregiver Santiago tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. LPA also observed several charged fire extinguishers mounted throughout the facility. The facility's certificate of liability insurance expires on 02/17/2027. Resident files reviewed had the Department's required records. Long Term Care Ombudsman's contact information, residents' personal rights, emergency disaster plan, complaint procedures and facility sketch are visibly posted in the hallway leading to the resident rooms. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Caregiver Santiago.
