Facility Evaluation Report
On 9/17/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required annual visit. LPA met with care staff, Latriece Scott, and explained the purpose of the visit. LPA conducted a tour of the facility and did not observe any health and safety concerns. During the time of visit, (3) three out of the (4) four clients were attending day program. (1) One client was present and interview during the time of visit. During today's visit, LPA Flores observed the following:
The physical plant is a two-story structure which consist of a dining room, kitchen, garage, (4) four client bedrooms, and (3) three bathrooms. Client bedrooms were equipped with the required bedding, furniture, seating, and functional lighting. All bathrooms were in safe and sanitary operating conditions. Additional linen and towels are available for clients in the laundry room and appear to be in good repair. LPA did not observe no pools or bodies of water. The facility met the (2) two-day supply of perishable food and (7) seven-day supply of non-perishable foods. Per Administrator, Lester Bell, there are no firearms or ammunition on the premises.
Indoor/outdoor passageways were free of obstructions. Outdoor patio area was observed to have shading and sufficient seating. LPA observed cleaning solutions, poisons, knives and other sharp instruments locked in a cabinet located in the garage. Water temperature measured at 114.9-degree Fahrenheit meeting within the department's requirements. Carbon monoxide and smoke alarm was tested and appeared to be operable.
