Facility Evaluation Report
On 05/19/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA was greeted and granted entry by Administrator Shawntae Nelson, where LPA explained the purpose of the visit. At the time of the visit there was (1) staff and (0) clients present, as they were at the day program. The facility has an approved fire clearance and is licensed to serve clients ages 18 to 59, and for (4) non ambulatory.
The facility is a single story structure that consisting of four (4) client bedrooms and two (2) bathrooms along with a kitchen, dining room, living room, laundry room, backyard with a covered patio and attached garage.
The hot water temperature in the client bathrooms was tested measured at 119.6 Fahrenheit. The facility was observed to have a 2-day supply of perishable foods and 7-day of non-perishable food items. The facility had an ample supply of paper goods and personal hygiene supplies.
The medications, chemicals, sharps and other hazardous items were observed to be locked and inaccessible to clients in care. LPA conducted a review medications for all (3) clients in care and the Medication Administration (MAR), which revealed the medications are being given according to the physician instructions. The smoke and carbon monoxide detectors were tested and observed to be operable. A records review was conducted of both staff and client files, the staff were observed to have valid CPR and crisis prevention intervention training. The Administrator Shawantae has a valid Administrator's certificate that expires on 01/08/26. LPA did not observe any deficiencies, therefore no citations were issued.
An exit interview was conducted and a copy of this report and LIC 811-Confidential names list was reviewed and provided to Administrator Shawntae Nelson.
