Facility Evaluation Report
Licensing Program Analyst (LPA) Andrew Martinez conducted an unannounced case management visit during complaint visit, control number 56-AS-20260602151340. LPA met with Administrator Emmanuel Oyebobola and explained the reason for the visit. Upon entry to the facility, there was (1) staff present.
During today's visit, LPA requested to review (4) of (4) client records. LPA observed client records to be incomplete or missing client medical assessments (e.g. Physician's Reports), and (1) of (4) client's records unavailable for inspection. Administrator explained to LPA that (1) of (4) client's file binder could not be located in the home and thinks it was taken with client to one of their appointments. This poses an immediate health, safety, or personal rights risk to persons in care.
Based on observations today, two (2) type A deficiencies were cited per Title 22, Division 6, of the California Code of Regulations.
An exit interview was conducted where this Facility Evaluation Report (LIC 809 and LIC 809-D) was discussed and a copy along with Appeal Rights, were provided to Administrator Emmanuel Oyebobola.
