Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced collateral visit to this community day program to conduct client interviews for investigation 56-AS-20250806145325, not related to this facility. LPA met with case manager, Noah Yaney and explained the nature of the visit.
During today's visit, LPA conducted interviews with Client 1, Client 2 and Client 3 ( C1, C2 and C3).
An exit interview was conducted and a copy of this report along with LIC811 was provided to Noah Yaney.
