The inspector’s account

On 08/13/2025 at 2:00 PM, Licensing Program Analysts (LPAs) LaVette Farlow and Renese Howell-Small conducted an unannounced visit to the facility to conduct the required comprehensive annual inspection to the facility. LPAs Farlow and Howell-Small were greeted by Daniel Musa and gained access at the home. LPAs explained the purpose of the visit over the phone to House Manager, Innocent Okwudifele.

The facility has four (4) bedrooms, three (3) bathrooms, kitchen, dining room, living room, attached garage, and backyard. The facility is vendorized by Inland Regional Center (IRC). The facility is licensed for 4 ambulatory residents. LPAs Farlow and Howell-Small completed a walkthrough of the facility, review of records, Personal and Incidental (P&I) and medications audit.

Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD), LPAs Farlow and Howell-Small observed zero (3) client during the visit. Staff Daniel Musa reported to LPAs Farlow and Howell-Small that one client is currently out of the facility on an appointment. There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 74 degrees Fahrenheit. LPA Farlow inspected client bedrooms; they are equipped with required furniture such as: mattresses, nightstands, storage space, chairs, and sufficient lighting. LPA Farlow inspected client bathrooms; bathrooms were clean, and appliances were found functional. Water temperatures tested at 111.7 and 108.5 degrees Fahrenheit. The facility is equipped with operational combined smoke detectors and carbon monoxide detectors, (2) charged fire extinguishers, and first aid kit with first aid book. In addition, LPAs Farlow observed non-slip mat on client’s bathroom.
