The inspector’s account

On 05/8/2026 at 08:50 AM, Licensing Program Analyst (LPA) Beena Singh conducted an unannounced visit to the facility to conduct the required comprehensive annual inspection. LPA Singh was greeted by a Staff/DSP-Grace Nwankwo and gained access at the home.Designee/Administrator Charles Okojie was contacted and informed of the visit. LPA Singh explained the purpose of the visit to Staff Grace Nwankwo and Designee/Administrator Charles Okojie who arrived during the visit.

The facility has four (4) bedrooms, two (2) bathrooms, a living room, den, kitchen/dining room, attached garage, and front/backyard. The facility is vendorized by Inland Regional Center (IRC). LPA completed a walk through of the facility, review of records, medications audit and Personal & Incidental (P&I) audit.

Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD). LPA Singh observed one (1) client who left for the day program during this visit. There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 76 degrees Fahrenheit. LPA inspected client bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, chairs, and sufficient lighting. LPA Singh inspected client bathrooms; bathrooms were clean, and appliances were found functional, water temperature tested at 122 degrees Fahrenheit. The facility is equipped with operational smoke detectors and carbon monoxide detectors, charged fire extinguishers, and first aid kit with first aid book.
