Facility Evaluation Report
Licensing Program Analyst (LPA) Hannah Rodgers conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was greeted by, identified herself to and discussed the purpose of the visit with Licensee Emmanuel Oyebobola. The facility's license shows a maximum capacity of four (4) ambulatory clients. During today’s inspection there were four (4) clients in care.
During today's visit, LPA observed a double-sided lock on the front door of the facility that can only be unlocked with a key from the inside and outside. There was no doorknob on the front door but rather it was replaced by the double-sided lock. LPA observed a staff have to unlock the double-sided lock with a key when someone knocked at the front door. Interviews with staff revealed that Client #1 (C1) is an elopement risk and that was the reason for the double-sided lock. Furthermore, LPA observed Client #2 (C2)’s bedroom window did not have a screen and Client #3 (C3)’s bedroom window screen was in disrepair. C3’s window screen had a gap that was sealed with duct tape. Interviews with clients revealed the duct tape was place on the window and screen to prevent outside elements from coming in.
LPA reviewed facility records. Due to time constraints, the annual inspection could not be completed and a return visit on a subsequent day is needed.
On today's date, two deficiencies were cited per California Code of Regulations Title 22 and noted on the attached LIC809-D pages. Additionally, since the deficiency was related to a fire clearance violation, an immediate Civil Penalty of $500 was also assessed on the LIC421IM. An exit interview was conducted with Licensee Emmanuel Oyebobola, whose signature below confirms receipt of a copy of this report and the Licensee Appeal Rights (LIC9058 3/22).
