Facility Evaluation Report
Licensing Program Analysts (LPA) Andrew Martinez made an unannounced visit to the facility to conduct the required annual inspection. LPA identified themselves, met with House Manager Muideen Salami and discussed the purpose of the visit. The facility is an Adult Residential facility with a license capacity of (4) and a current census (2). LPA conducted an overall inspection, which included, but was not limited to, the following:
Operation/Physical Plant: Indoor and outdoor passageways were kept free of obstruction. The facility has no swimming pools or similar bodies of water. Client outdoor activity space is shaded and enclosed with self latching gates. The facility is equipped with smoke/carbon monoxide alarms in working condition, laundry equipment, and hallway emergency lighting. Client bedrooms were equipped with beds, nightstands, chairs, and lighting. Client bathrooms were operating in safe conditions. The hot water in client bathrooms tested at 110 degrees F. Sharps, disinfectants, cleaning supplies were kept locked and stored away from food supply. The facility has 24 hour/7 days a week care staff. LPA observed fireplace not made inaccessible to clients, facility is equipped with telephone service but does not currently have a telephone in the home; and stairway post/hand railings and various doors and windows are in disrepair; technical deficiencies are being cited.
Food Service: The kitchen and dining area was maintained clean. The facility has sufficient non-perishable and perishable food for number of clients in care.
Health Related Services: The facility maintains medications in a locked cabinet. LPA observed both clients Medication Administration Records (MARs) to be accurate in accordance with dispensed medication to clients.
