Facility Evaluation Report
Licensing Program Analyst (LPA) Armando Perez conducted an unannounced Case Management- Deficiencies visit. LPA met with Administrator Nicole Lara who assisted with the visit.
During the facility inspection, LPA observed two holes in the wall that will need repair. One is located in the hallway and measures a foot length and 8 inches height. A second hole was discovered in a bedroom measuring 3 by 3 inches. Administrator stated that a work order had been placed and will be repaired this week or early next week.. A deficiency will be issued and a plan of correction will be required.
An exit interview was conducted, and a copy of this report, 809-D and Appeal Rights was discussed and provided to facility representative.
