Licensing Program Analyst (LPA), Stephanie Torres, conducted an unannounced visit to the facility to conduct a health and safety visit in relation to complaint #18-AS-20211220091157. The LPA met with Interim-Administrator, Ruth Fromme, and informed her of the purpose of this visit.
No health and safety concerns were observed during this visit. This report was reviewed with Fromme and a copy was provided.
