Facility Evaluation Report

On Saturday, 07/18/2026, Licensing Program Analyst (LPA) Jacqueline Shaw-Ross conducted an unannounced case management visit to the facility to assess for any health or safety concerns. LPA met with Joselynn Martinez, Medical Technician and was informed of the purpose of the visit.

Upon arrival, LPA observed residents participating in social activities in the main lobby. LPA conducted a tour of the facility in buildings A, B, C and D, including a random sample of residents' bedrooms across all facility buildings. Based on the observations made during today's visit LPA did not identify any health or safety concerns and no deficiencies were cited.

An exit interview was conducted and a copy of this report was reviewed and provided to Joselynn Martinez, Medical Technician.
