Complaint Investigation Report
Regarding the allegation, "Residents are left in soiled diapers for an extended period of time ". Through review of documentation, and interviews conducted with residents and staff, issues related to residents not being regularly changed related to incontinence was found to not occur. LPA also decided that due to the lack of evidence to corroborate the allegation, the allegation was deemed to be UNSUBSTANTIATED. A finding of UNSUBSTANTIATED means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
An exit interview was conducted and a copy of this report was discussed with and provided to Ms. Mapilis along with copies of the LIC9099-C, and LIC811.
