Facility Evaluation Report

On 11/10/2025, Licensing Program Analyst (LPA), Javina George made an unannounced visit to the facility to conduct a 1 year required inspection. LPA was greeted and granted entry by Administrator Rosy Ybanez who was informed of the purpose of the visit, and accompanied LPA throughout the inspection. LPA verified facility contact information on file, and will update accordingly. The facility is licensed to serve and has an approved fire clearance for four (4) ambulatory clients age range 18 through 59. At the time of the visit there was (1) staff and (0) clients present as there are currently no clients in care.

LPA conducted a tour of the interior/exterior areas of the facility. LPA observed for the home to be equipped with solar panels, and to have video cameras installed. The facility was observed to have the required postings and to be clean and clutter free, with the passageways being free of any obstruction. There are no pools or bodies of water on the premises. The client bedrooms were observed to have the required furniture (bed, lamp, chest of drawers, chair).

LPA observed two (2) fully charged fire extinguishers, that were last serviced on 10/14/25.The smoke and carbon monoxide detectors were tested and found to be operational. The hot water temperature in the bathrooms measured to be 113.3 degrees Fahrenheit. The facility was observed to have emergency food and water. An ample supply of personal hygiene supplies, towels, linen and games.

Administrator Ybanez was observed to have a valid administrator certification and CPR certification.

There were no citations issued during today's visit.

An exit interview was conducted and a copy of this report was reviewed and provided to Administrator, Ybanez.
