Facility Evaluation Report
On 11/27/2024, Licensing Program Analyst (LPA), Janette Romero arrived unannounced to conduct an annual required inspection. LPA was greeted and granted entry by Caregiver, Mayra Zavaleta who was informed of the purpose of the visit. Administrator, Rosy Ybanez arrived during the visit. The facility has a fire clearance for four (4) ambulatory clients and serves adults ages 18 through 59.
LPA toured the facility's interior and exterior with Administrator Ybanez. During the tour, LPA observed the facility is made up of a one (1) story home with four (4) client bedrooms and two (2) bathrooms along with a kitchen, dining room, living room and attached garage. There are no bodies of water on the premises. Indoor and outdoor passageways are free of obstruction. An outdoor shaded seating area is available. LPA observed two (2) charged fire extinguishers mounted in the facility. Administrator Ybanez tested one (1) of the smoke alarms/carbon monoxide detectors and LPA observed it to be operational. LPA observed kitchen area to be clean and the facility had a two-day supply of perishable foods and seven-day of non-perishable food items. Food is stored in a safe and healthful manner. Medications were secured in a hallway closet. LPA reviewed Client 1's (C1's) medication administration record while Administrator Ybanez reviewed their physical medications and no discrepancies were discovered. LPA reviewed C1's file which had an updated Individual Program Plan and signed admission agreement. Client bedrooms were each furnished with a bed, chair, closet, clothing storage and lighting. Bathrooms have a working toilet, wash basin along with grab bars and non-skid mats in the shower. The hot water temperature in the bathrooms measured at 116- and 117-degrees Fahrenheit. Administrator Ybanez reported the facility does not safeguard C1's cash resources. Staff present has a criminal record clearance and valid first aid/CPR and nonviolent crisis intervention certification. The facility also has a valid certificate of liability insurance. Complaint procedures, facility sketch, and license are visibly posted near the front entrance. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Administrator, Ybanez.
