Licensing Program Analyst (LPA), Seo Jeon, conducted an unannounced visit to the facility to deliver amended findings of complaint report number 18-AS-20220404101121. The LPA was allowed entrance into the facility and met with Maria Forkrud, Administrator. The LPA informed the Administrator of the purpose for the visit.
LPA toured the interior and exterior of the facility. LPA did not find any concern for health and safety of the residents in care.
Amended findings were delivered and an exit interview was conducted and a copy of this report was provided to the Administrator.
