Facility Evaluation Report
On today's date, 09/16/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management incident visit. LPA was greeted and granted entry by House Manager Belen Estrada where LPA explained the purpose of the visit. The administrator Cheryl Balquiedra was available via telephone.
On 09/10/25 the department received an incident report, informing of an incident involving Client #1 (C1) that occurred on 09/05/25. C1 was reported to be upset and began demanding money while waiving a pocket knife at Client #2 (C2) and facility staff.
During today's visit LPA obtained updated contact information for the facility, conducted a tour of the interior/exterior areas of the facility, checked food supply, as well as conducted interviews and records review. Per an interview conducted via telephone with Administrator Cheryl Balquiedra, staff were not aware of C1 having a pocketknife in their possession. The pocketknife was retrieved by law enforcement on 09/05/25. At this time the administrator has reported that the facility is willing to continue to work with C1.
There were no health and safety concerns observed during today's visit. However note that LPA was unable to enter two of three client bedrooms as the clients do not wish to have anyone enter their room without their permission. LPA observed for there to be a hole in the staff bedroom door (E). Per house manager the administrator is having the door repaired. Per a review of daily notes, the incident occurred on 09/05/25. There were no citations issued during today's visit.
An exit interview was conducted where a copy of this report was reviewed and provided to Belen Estrada, house manager.
