Facility Evaluation Report

Licensing Program Analysts (LPA)’s Armando Perez and Robert Campbell, conducted an unannounced visit to the facility to conduct a Plan of Correction (POC) Visit. The purpose of this report is to document the POCs that were previously agreed upon. LPAs met with Administrator, Rolanda Henderson who was informed of the purpose of the visit. LPAs conducted a walk through, conducted interviews and records review.

During Annual Visit conducted on 12/05/2025 the facility was cited for California Code of Regulations (CCR) Section 80092(b)(1) for not having an exception for a restricted health condition. The POC was to submit documentation that accompanied an exception request by the POC due date of 01/02/2026.

LPA obtained documentation that accompanied an exception request. LPA will review and provide a response in a timely manner.

There were no deficiencies and no civil penalties that were cited per Title 22, Division 6, of the California Code of Regulations.

An exit interview was conducted, and a copy of this report was discussed and provided to facility representative.
