The inspector’s account

On 5/20/2026, Licensing Program Analyst (LPA) Mia Lankford conducted an unannounced visit to the facility to conduct an Annual inspection. LPA was greeted and granted entry by Administrator Frank DeAguilar who was informed of the purpose of the visit. At the time of the visit, LPA was notified that there was four (4) clients in care and three (3) staff on duty.

LPA toured the facility with Administrator Frank DeAguilar and reviewed staff and clients records. During the tour, LPA observed the facility is made up of a one (1) story home with four (4) client bedrooms, one (1) office room, three (3) client bathrooms, two (2) living rooms, a dining room, kitchen and attached garage. All client bedrooms had required furniture and lighting. LPA toured the facility's exterior and observed outdoor pathways were free of obstructions. Outdoor shaded seating area is also available for the clients in care. LPA toured the kitchen and observed the facility has a 2-day supply of perishable foods and a 7-day supply of non-perishable foods, which are stored in a safe and healthful manner. LPA toured the garage and observed an additional refrigerator and freezer with perishable foods. LPA also noted the facility secures additional cleaning solutions, laundry detergent, and disinfectants in a locked cabinet in the laundry room. Administrator tested one (1) of the smoke alarms/carbon monoxide detectors and LPA observed it to be operational. LPA also observed two (2) charged fire extinguisher mounted near the kitchen area, last serviced on 10/13/2026. Medications are secured in a locked cabinet stored in the office room.
