Facility Evaluation Report
Licensing Program Analyst (LPA) Armando Perez conducted an announced visit for the purpose of confirming the recent changes to the facility. LPA received an updated approved fire clearance, conducted by Hemet Fire Department on 12/18/2025, and an updated facility sketch. The facility change now has the bedroom 2 and 4 approved for non-ambulatory clients. The capacity will remain the same at four (4) which now two (2) may be non-ambulatory.
LPA was greeted and granted entry by Administrator Kyle Almendares, who also has a current administrator certificate that expires on 07/18/2027. LPA conducted a tour of the interior and exterior of the single story home which consists of 4 bedrooms, 3 bathrooms, each bedroom was fully furnished. The home was clean, clutter and odor free. LPA observed for the facility food supply to meet the minimum requirements of having a 2 day supply of perishable and 7 day supply of non-perishable food items.
No health and safety concerns were observed during today's visit.
An exit interview was conducted, and a copy of this report was provided to facility representative.
