On 07/31/2026, Licensing Program Analyst, (LPA) Ahliah Sharp arrived unannounced at the facility to conduct a Health and Safety visit. LPA met with Administrator Andrea Scott and explained the purpose of the visit.
LPA toured the facility and observed the food level to be outside of the recommendation per Title 22; a deficiency is cited on the attached LIC 809-D.
An exit interview was conducted, and a copy of this report and Appeal rights were provided to Administrator Andrea Scott.
