Facility Evaluation Report

On 10/13/2025, Licensing Program Analyst (LPA) Armando Perez conducted an unannounced visit to the facility for the purpose of issuing citations for deficiencies observed during the investigation into Complaint Control No. 18-AS-20250520150021. LPA met with Administrator Andrea Scott and explained the purpose of LPA's visit.

During facility records review, LPA observed the facility had not submitted an incident report relating to the allegations of a malfunctioning water heater that left residents in building 2 without access to hot water throughout the day on multiple occasions. Based on record review and interviews, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. See LIC809-D.

An exit interview was conducted and a copy of this report and appeal rights was discussed with and provided to Administrator Andrea Scott.
