Facility Evaluation Report

Licensing Program Analyst (LPA), Stephanie Martinez, conducted an unannounced visit to the facility to follow up on an alleged violation reported to the LPA. The LPA met with Facility Manager, Breanna Bridges, and informed her of the purpose for her visit.

Reports were obtained from staff and clients regarding staff members cussing at or around clients in care. Four (4) staff members reported staff members do cuss at or around clients and have done so on multiple occasions. Two (2) clients also reported staff do cuss at clients in care. This poses a potential threat to the personal rights of the clients in care. Therefore, based on interviews, a citation will be issued.

An exit interview was conducted; this report was reviewed with Facility Manager, Breanna Bridges, and a copy was provided, along with instructions on appeal rights.
