Facility Evaluation Report
On 08/21/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA met with Levi Michaud, administrator and explained the purpose of the visit. The facility has not had any clients since becoming licensed on 08/21/24. Per Levi the facility is currently awaiting to become vendorized with the placement agency. LPA verified contact information, with no changes. Below are the observations made during today's inspection.
The facility was clean with the passageways being free of obstruction. The facility is a single story structure consisting of 4 client bedrooms, 2 bathrooms, laundry room, living room, dining area, kitchen backyard and garage. Client medication will be centrally stored and locked in the kitchen cabinet. There are no pools or bodies of water on the premises or guns and ammunition. The combined smoke and carbon monoxide detectors were tested and found to be operable. The hot water was tested and found to be within regulatory limits, measuring to be 109.2. There is plenty of space with indoor and outdoor activities to encourage and promote socialization. There is a full pickle ball court, and fire pit with plenty of seating in the backyard.
The rooms were observed to have the required furniture. Facility met the required 2-day supply of perishable and 7-day supply of non-perishable food items. All staff present were observed to have obtained criminal record clearance and to be associated to the facility. The administrator Levi was observed to have a valid administrator certification that expires on 02/26/26, and valid Cardio Pulmonary Resuscitation (CPR) and first aid certification that expires on 05/24/26. The facility licensing annual fees that were due today, were paid during today's visit.
Based on todays inspection, no deficiencies were observed. An exit interview was conducted and a copy of this report was reviewed and provided to Levi Michaud, administrator.
