Facility Evaluation Report
On 4/8/2026 Licensing Program Analyst (LPA) Mia Lankford met with Administrator Cristhian Rodriguez and explained the reason for the unannounced Case Management Visit.
The LPA observered that four (4) clients currently reside at this facility, ten (10) staff is currently present.
During this visit LPA toured the facility on the inside and outside. LPA assessed the food supply and found that it exceeds the minimum requirement of a 2-day supply perishable and a 7-day supply of non-perishable foods. Utilities are on and functioning, LPA observed sufficient staff for care and supervision of clients.
Based on the information obtained today, LPA does not observe any immediate threat to the health, safety, and welfare of the clients residing at this facility. There are no deficiencies being issued per Title 22, Division 6 of the California Code of Regulations.
This report was reviewed with Administrator Cristhian Rodriguez and copy provided to the facility representative at the time of exit interview.
